Dear Prospective Intended Parent:
Our Fairy Godmother LLC is pleased to provide you with our 2010 Stimulus Package. We appreciate how expensive egg donation has become and have lowered our fees by almost 40% which offers a significant savings of $3,000.00 from our previous fees for our local donors and a 25% savings of $2,250.00 for all our donors who require air travel. Please find an estimation of fees you can anticipate at www.ourfairygodmother.com/fees
Please note these fees do not include the donor’s reimbursement. The donor’s fees are listed on their profiles. Our fees also do not include any clinic fees or medications – you will be invoiced directly from your fertility clinic for their services. Please read the area entitled -Additional Expenses Not Included in the Flat Fees for further details.
If you have not yet registered on our website, go to www.ourfairygodmother.com and complete the form “Becoming an Egg Donor Recipient”. Once you have completed the form, we will issue you a username and password which will come by email. Take a look at the donor profiles and photos, and then we can also arrange a complimentary one hour telephone or Skype webcam consult (www.skype.com) to help narrow down your choice and then go over the next steps. Once you have logged onto our website, you may view all of our egg donor profiles and their photos at your leisure. There is a button for you to save a donor to a favorite's folder so you can refer to it again in the future.
We currently have over 180 outstanding donors in our database and have new donors joining our program daily. We have donors in 32 states and 9 Canadian provinces!

FEE SCHEDULE – MARCH 1, 2010
Below please find an estimation of fees you can anticipate to pay for an egg donation cycle for the four different egg donation options
Option One $5,000.00 – Shared Cycle with a Donor Who Does Not Require Air Travel
All of the retrieved eggs are shared between two recipients. The savings in this option is that you share the all the fees related to the donor’s treatment.
The previous fee for Option One was $8,000.00 plus the donor fee.
A $3,000.00 stimulus savings is offered!

The previous fee for Option Two was $8,500.00 plus the donor fee.

- Cycle Monitoring at a satellite clinic – includes blood tests and ultrasounds and related clinical costs for the donor to be monitored at a clinic near her home prior to travel up to three times (this fee does not include initial or subsequent donor screening prior to commencing a cycle as these tests are usually ordered by the recipients attending physician and may vary) – shared
- Travel expenses for the donor and her companion including airfare, hotel accommodations, meal allowance and transportation to and from the clinic and hotel/airport for the donor for the egg retrieval for up to five nights. This fee does not include any travel that the donor may require for screening as some clinics require a donor to be seen for initial/follow up screening and physical exam onsite prior to the cycle commencing). Nor does it include airline change fees or additional days required for hotel, meals and childcare if she is required to stay more than five days - shared

of $2,500.00 is $11,500.00+
Option Four - $11,700.00 – Solo Cycle with a Donor Who Requires Air Travel
The previous fee for Option Four was $13,950.00 plus the donor fee.

- Cycle Monitoring at the satellite clinic – includes blood tests and ultrasounds and related clinical costs for the donor to be monitored at a clinic near her home prior to travel up to three times (this fee does not include initial or subsequent donor screening prior to commencing a cycle as these tests are usually ordered by the recipients attending physician and may vary)
- Travel expenses for the donor and her companion including airfare, hotel accommodations, meal allowance and transportation to and from the clinic and hotel/airport for the donor for the egg retrieval for up to five nights. of $5,000.00 is $16,700.00+
ADDITIONAL EXPENSES THAT ARE NOT INCLUDED IN THE FLAT FEES
We do not charge any additional agency fee to coordinate these additional charges listed below
- Donor Reimbursement – Donor fees vary from $5,000.00 to $10,000.00. A first time donor receives $5,000.00 however donor who are proven experienced egg donors who have undergone more than one cycle may request a higher fee. The average donor fee is between $5,000.00 - $7,500.00. These fees are listed on each donor’s individual profile in the Directors Comments.
- Travel expenses - If your donor lives more than 1.5 hour’s driving distance from the clinic but does not require air travel she will require additional travel expenses. Travel costs for these donors is $0.505 cents per mile each way between the donors home and the fertility clinic for screening, cycle monitoring and egg retrieval/follow – up as calculated by MapQuest, meal allowance of $50.00 per day if alone $75.00 per day if she is required to have a companion, $50.00 per day childcare expenses and reasonable hotel accommodations if she is required to spend the night.
- Donor initial or repeat infectious disease FDA screening – These fees are billed to the recipient directly by ReproMed – The Toronto Institute of Reproductive Medicine as Dr Del Valle will determine what he would like the donor to have done in accordance with federal laws. Our Fairy Godmother does not provide infectious disease screening. This is the responsibility of the Intended Parents fertility specialist to provide doctors orders for the donor to undergo FDA lab testing.
- Donor travel for screening – ReproMed – The Toronto Institute of Reproductive Medicine requires a donor they have not yet cycled in the past to attend one screening appointment so they can personally meet her, perform a physical exam, blood work and ultrasound and meet the nurses, tour the facility and undergo teaching prior to the cycle. We will coordinate these appointments and make all the travel arrangements however you will be billed extra for the travel costs (i.e., transportation, meal allowance, childcare etc).
- Additional travel expenses for the donor and her companion including airfare, hotel accommodations, meal allowance and transportation to and from the clinic and hotel/airport for the donor for the egg retrieval. Satellite clinic visits for blood work and ultrasound. Additional days required for hotel, meals, childcare that is not included or over and above what is listed in your Fee Option.
- MMPI Psychiatric Testing – All of our donors undergo psychiatric testing which is included in the flat fees listed above. However if you require an MMPI – you can anticipate it costing approximately $1,150.00
- Criminal Background Check – if requested, we will perform a Criminal Background Check on your donor and provide you with a report (excluding her identifying information). The fee is $250.00
- IQ Testing - $350.00
- Passport fees if the donor is required to obtain a passport for she and her companion to travel for screening or retrieval if they do not already have one or their passports require renewal
- Other expenses such as fertility medications, physician’s fees and treatment fees for the donor and recipient IVF treatment are not included in our fees as they will be invoiced to the recipients directly by their fertility clinic. You should discuss these fees for a shared and solo cycle with your physician.
- Any travel or travel related expenses the recipients may require
- Any medical treatment, tests or medications the recipients may require
- Many of our recipient families have elected to send their donor a special thank you gift. Often a gift of flowers or a special card is a nice way to say Thank You! Our staff will be happy to assist you in coordinating this thoughtful gesture at your request. You just let us know what you would like us to do; we’ll bill you for it and send the gift to the donor.
Please note the only services we provide and are invoicing you for are those listed in our fee schedule. Any other fees or services should be discussed with your clinic.
All fees and services are subject to change without notice.
PAYMENT SCHEDULE
Fees are paid to the agency in two installments. The first fee of the option you have chosen plus half of the donor’s fee is due upon signing our Agency – Recipient Agreement. The second installment which is the second half of the donor’s fee plus any outstanding additional fees is due at least three business days prior to the donor commencing her first injectable medication. Fees for Additional Expenses that may be incurred must be paid in full prior to the expense being incurred.
REFUND SCHEDULE
If OUR FAIRY GODMOTHER, LLC., is unable to cycle the chosen donor within 6 months with a medically suitable Donor meeting clients general physical description, as defined in writing by clients, then clients shall be credited a portion of fees paid as stated herein, but shall remain responsible for all out-of-pocket costs incurred to locate Donors for client and a fee of $4,500.00 for OUR FAIRY GODMOTHER, LLC.,’s time and services. All refunds are based on the Donor Agreement not being drafted or credits. No other refunds shall be given. If a subsequent donor is chosen Our FAIRY GODMOTHER, LLC will provide services for a new cycle at a reduced fee of $3,500.00.
