STEPS IN BECOMING A RECIPIENT

1. Contact our program by calling 1-877-240-4351 or by emailing us at dreams@ourfairygodmother.com. We would be happy to provide you with additional information about our donor program and explain the various categories of donors and fees. 2. Complete a 5-page recipient profile about yourself and your spouse or partner. You can submit this form by clicking Recipient Profile.

3. Once your recipient profile is received, you can request additional information about particular donors, including pictures if available. If necessary, we will perform a donor search when specific cultural or genetic traits are desired. After browsing our online donor database, intended parents select potential egg donors, and retain Our Fairy Godmother services. The next step is to narrow the number of potential donors, finally choosing one donor with whom to proceed. The process can take a matter of days or weeks, depending on each individual case.

4. When you have decided upon a particular donor, we will contact her to make sure she meets your timeline and is available to travel to your designated clinic.

5. At this point you will need to sign our Recipient – Agency agreement with our agency that outlines the costs involved and you will be emailed an invoice for the complete cycle not including any of the clinical medical treatments that you will be billed directly from your fertility clinic. A non-refundable deposit of $500 is required. The deposit will be applied to the agency fee once you have contracted our services. You will also be required to pay at least %50 of the donors fee and her expenses at that time. Any outstanding balances must be paid prior to the donor commencing her first injectible medication.

6. You will need to have all contracts in place prior to any donor stimulations. We will coordinate all legal contracts on yours and the donors behalf and send them to you from the lawyer. We will pay the attorney directly.

7. The chosen donor is then comprehensively screened medically by your fertility clinic and we undertake the psychological screening, a process that usually takes two to four weeks. Once this is complete, the intended parents enter into a contract with the egg donor. This process takes another two weeks on average. The donor will begin her medications, a process lasting approximately five weeks, culminating in egg retrieval. The embryos are then ready for transfer to a surrogate or the intended mother, or the embryos may be frozen for use at a later time.

If I choose a donor from Our Fairy Godmother, can I still use my own doctor?

ANSWER: Yes. After you choose a donor, she can be sent to any qualified physician of your choice to proceed with the required medical treatment.

Are most of the donors in your registry college educated?

ANSWER: Yes, the majority of donors in Our Fairy Godmother database are college educated or currently attending college. GPA's, SAT or ACT scores are included in each donor's profile and, for an additional fee, IQ testing is available.

What are the ages of the donors in your agency?

ANSWER: Our donors are between the ages of 21 & 35 years old.

Are your donors medically screened?

ANSWER: All of our donors are thoroughly screened for communicable diseases in accordance with the FDA/ ASRM and when applicable the country where the donation and transfer will take place if outside of the US. Your fertility clinic will request and conduct all the testing they require by law. Our Fairy Godmother is not a medical clinic and cannot order tests to be conducted. Those tests can only be ordered by a licensed physican.

Are your donors psychologically screened?

ANSWER: Yes, all donors are required to have a thorough psychological evaluation or by a psychologist at your fertility clinic.
